About Colorado Access Health Plan
Effective March 1, 2010, all members of Colorado Access Health Plan - Enhanced Care Management were transitioned to Colorado Access Enhanced Primary Care Case Management through the State's passive enrollment process.
In 2008, Colorado Access partnered with the Center for Health Care Strategies, and the State of Colorado Department of Health Care Policy and Financing to form a plan specially designed for disabled and elderly Medicaid recipients. Colorado Access Health Plan - Enhanced Care Management is available to individuals that reside in Adams, Arapahoe, Boulder, Broomfield, Denver and Weld Counties and who receive Medicaid benefits through the Disabled (AND) and Elderly (OAP) aid categories.
What is Enhanced Care Management?
Enhanced care management is intended to support a member's health care experience with involvement of care managers and providers. Additional program goals include:
- Improve the quality of care offered to members.
- Decrease the cost of care for Medicaid clients.
- Provide a medical home to patients that do not have one.
