Access Food for Shots

Your child’s health is very important to us!  One of the ways to help your child stay healthy is to make sure they are up-to-date on their immunizations (shots). 

If your child is between the ages of 12 and 24 months and enrolled with CHP+ offered by Colorado Access, send us proof that they are up-to-date on their shots and we’ll send you a $10 grocery gift certificate! 

Not only will we send you the $10 grocery gift certificate, we’ll also enter your family’s name into our annual drawing for a $250 grocery gift certificate!  The drawing takes place at the end of the calendar year, just in time for the holidays.

How do I get my $10 grocery gift certificate?

• Click here to download and print our Food for Shots brochure (PDF).* 
• Once your child is up-to-date with his or her shots, fill out the form located on the brochure.  
• At your child’s next appointment, have their provider sign it.  
• Remember, your child must be up-to-date on their shots, enrolled with CHP+ offered by  Colorado Access at the time we receive the form, and between the ages of 12 and 24  months.  Mail the completed form to:

• Please submit one entry per child. 

* If you cannot print this brochure/form, you can mail us your child’s shot record that shows they are up-to-date.  To get a copy of the shot record, ask your child’s provider.

Once we receive proof that your child’s shots are up-to-date, we’ll send you the $10 grocery gift certificate and automatically enter your family’s name into our annual drawing for a $250 grocery gift certificate.
