To Get Your $10 Gift Card: Get your Annual Wellness Exam as soon as possible. Have your provider sign your coupon. Then, send the coupon to Colorado Access.

Mammogram Colorectal Cancer Screening
One mammogram is covered each year for women ages 40 and over, or who have a family history of breast cancer. Talk to your provider if you think you need a mammogram or if you are not sure.

To Get Your $10 Gift Card: Get your mammogram. Have your provider sign your coupon. Then, send the coupon to Colorado Access. If you’ve already had your mammogram, have your provider sign the card letting us know you are up to date.

 One colorectal cancer screening is covered each year for members who are ages 50 and over, or for members who have a family history of colorectal cancer. Talk to your provider if you think you need to get a screening or if you are not sure.

Annual Eye Exam
One eye exam, by an eye professional (for glaucoma screening), is covered per year for members who are ages 50 and over, or at high risk. Talk to an in-network eye professional if you think you need to get a screening or if you are not sure.

Enrolled members are eligible to receive each Prevention Perk once a year. The Perks are of $10 value. You may not receive more than $50 in Perks each year. Please allow up to 6 weeks to receive your Perk. We reserve the right to verify that you are eligible for the Perk before we send it to you.
