Elevate Medicaid Choice

If you live in Adams, Arapahoe, Denver or Jefferson counties, you can enroll in the Elevate Medicaid Choice plan. With this plan, Elevate Medicaid Choice manages your physical health benefits. That means that you can get physical health care at the Denver Health main campus. You can also go to any of Denver Health’s nine family health centers in metro Denver. You can also go to Denver Health school-based health centers.
