We're Here to Help

Once you qualify for Health First Colorado (Colorado’s Medicaid Program) long-term services and supports, you have many benefits. These include:

Home & Community Based Services (HCBS):

This may include:

  • Transportation
  • Adult day and respite programs
  • Non-medical care to help with housekeeping and personal care

Nursing Care Communities/Skilled Nursing Facilities:

This may include:

  • Medical care
  • Post-acute care
  • Therapeutic services
  • Short-term rehabilitation
  • Long-term chronic care

Program for All-Inclusive Care for the Elderly (PACE): This program coordinates all long-term services and supports for you. We can help you decide if this is the best choice to help you stay in your own home.

Home Care Allowance (HCA): This provides a small payment for in-home services from a provider you choose.

Assisted Care Facility (ACF) also known as Assisted Living: This is a system of housing and limited care. This is a good option if you need help with daily activities but do not need a nursing home.

We can help manage the services you qualify for and need. We can also help coordinate them. Our goal is to help you live a more independent life.
