Child Health Plan Plus (CHP+)

Child Health Plan Plus (CHP+) offered by Colorado Access is a low-cost health insurance plan for children through age 18 and pregnant people.

Applying for CHP+

When you apply for health benefits, they will check if you qualify for Health First Colorado (Colorado’s Medicaid program) or CHP+. To apply for benefits:

Referrals and Prior Authorizations

You do not need a referral to get care from any in-network provider. But some services require prior authorization. Your provider must ask for approval for you to get these services. If you have questions about prior authorization, ask your provider or call us at 800-511-5010.

Enrolling in CHP+ offered by Colorado Access

You are automatically enrolled in a CHP+ health plan based on where you live. If you live within our service area and are not enrolled in CHP+ offered by Colorado Access, you may change your health plan within 90 days after you are first approved for CHP+. Getting Your CHP+ ID Card

Bring your CHP+ ID card when you need medical care and have it ready when you make an appointment. If you did not get your ID card or need a new ID card, call us. CHP+ ID cards are not yet available online.

When You Turn 19

CHP+ insurance ends on your 19th birthday. But we can help you find health insurance! Request an appointment with our application assistance site or call 855-221-4138.

CHP+ Benefits

Below is a summary of CHP+ benefits. Check the CHP+ Member Handbook / Folleto de los Beneficios del Miembro para CHP+ for more information.

Outpatient physical rehabilitation (physical, occupational, and speech therapy). CHP+ offered by Colorado Access members get 10 extra outpatient physical rehabilitation visits, for a total of 40 visits per diagnosis each year.

  • Preventive care (well visits, vaccines/shots)
  • Family planning and reproductive health benefits
  • Routine sick visits and specialist care
  • Hospital
  • Lab and X-ray services
  • Urgent and emergency care
  • Mental health and substance use disorder treatment
  • Ambulance transportation for emergencies
  • Prescriptions/pharmacy and some over-the-counter medications. See CHP+ Pharmacy Services below to learn more.
  • Durable medical equipment (DME) (ex. wheelchair, breast pump, diabetic strips)
  • Vision services. See CHP+ Vision Services below to learn more.
  • Dental care. Call DentaQuest at 888-307-6561 or use State Relay 711 for help.

CHP+ Pharmacy Services

Your CHP+ pharmacy benefit covers many prescription medications and some over-the-counter medications. Some medications may have coverage guidelines, such as prior authorization or quantity limits. Find out what medications we cover.

You can go to any in-network pharmacy to get your prescriptions.

Navitus Health Solutions manages your CHP+ pharmacy benefit. Sign up for the Navitus member portal here.

If you had to pay for a prescription after you qualified for CHP+, but before you enrolled with Colorado Access, ask your pharmacy for a refund. You must ask for the refund within 120 days from the date the prescription was filled. Give your CHP+ ID number and the pharmacy information below. Refunds are not guaranteed.

We may be able to reimburse you if you pay for a covered prescription at an in-network pharmacy if you did not use other insurance or a discount card. You must request reimbursement within 120 days of filling the prescription. Get an itemized receipt and send it with the Prescription Drug Claim Form to:

Members who need a 90-day supply of medication can have their prescriptions mailed through Birdi, our network mail-order pharmacy (Click here to enroll) or by asking your provider to write your prescription for up to a 90 day supply and present the prescription at an in-network pharmacy. Certain medications, including controlled substances and specialty medications are not be eligible for a 90 day supply.

CHP+ Vision Services

CHP+ offered by Colorado Access members get one routine eye exam and $150 toward eyeglasses or contact lenses each year. To use your benefits:

If you find a provider out of our network, you may purchase your eyeglasses or contacts and submit a receipt for reimbursement for up to the $150 annual benefit maximum. To request reimbursement for eyeglasses, call our customer service team at 800-511-5010 (TTY users should call 888-803-4494) or complete a grievance form. In the description box, put “eyeglass reimbursement” and attach your receipt.

  1. Go to an in-network provider for an eye exam.
  2. Take your vision prescription to an in-network provider who sells eyeglasses or contacts.
  3. If you find a provider out of our network, you may purchase your eyeglasses or contacts and submit a receipt for reimbursement for up to the $150 annual benefit maximum. To request reimbursement for eyeglasses, call our customer service team at 800-511-5010 (TTY users should call 888-803-4494) or complete a grievance form. In the description box, put “eyeglass reimbursement” and attach your receipt.

Finding a Doctor and Getting Appointments

To find a doctor or other provider, use our provider search tool or Health First Colorado Find A Doctor search tool. To find a dentist, use the DentaQuest Find A Dentist search. We can also help you find a provider. Call us at 800-511-5010. TTY users should call 888-803-4494.

You should be able to get appointments with in-network providers in these timeframes:

  • Urgent care: Within 24 hours of asking for an appointment.
  • Emergency mental health or substance use treatment: By phone, within 15 minutes after contacting the provider; in person, within one hour of contacting the provider.
  • Non-urgent sick care visit: Within seven days of asking for an appointment.
  • Non-urgent, symptomatic mental health/substance use treatment visit: Within seven days of asking for an appointment.
  • Well-care visit: Within one month of asking for an appointment.
  • Doctor visit after you leave the hospital: Within seven days of leaving the hospital.

CHP+ Copays

There is no premium for CHP+ offered by Colorado Access, but some services may have a copay. A copay is an amount you pay each time you get a specific service or prescription medication.
