Do you know someone who embodies compassion, dedication, and service to their community? We are accepting nominations for our 2025 Caring Heart Award! It’s our way of recognizing members of Health First Colorado (Colorado’s Medicaid program) and Child Health Plan Plus (CHP+) who demonstrate passion, dedication, and service to their communities.

What is the Caring Heart Award? The Caring Heart Award celebrates Health First Colorado and CHP+ members 16 and older who exemplify passion and demonstrate care through their volunteer work, service to our company, and contributions to community-based organizations. It’s our way of honoring those who advocate for improvements in the health care system and make a positive impact on their communities.

Nominate Someone Today! Do you know a Health First Colorado or CHP+ member who is 16 and older and deserves recognition for improving community health or health care? Nominate them for the Caring Heart Award! Nominations are open now and will be accepted until Friday, October 31, 2025.

Criteria for Nomination:

Demonstrates passion and dedication to their community.
Makes a positive impact on their community through their actions within the health and health care space.
Engages in volunteer work or service with Colorado Access.
Advocates for improvements in the health care system and Health First Colorado or CHP+ members.
