This form is used by providers to submit disputes regarding claims with Colorado Access. It requires detailed information about the claim, including the provider’s information, the member’s information, and the reasons for the dispute. Complete documentation, such as the claim and Explanation of Payment, must accompany the request. Each request must be submitted separately for each recipient and claim. The form can be sent via mail or email to the appropriate department.
