Designated Client Representative Form

The Designated Client Representative Form allows individuals to appoint a representative to manage appeals or grievances on their behalf. This form must be completed accurately to be valid and grants access to Protected Health Information (PHI). The authorization from the client expires after the matter is concluded or upon request.
