Get Help with Finding Care or Managing Your Care
We want to make sure you get the care you need. Our care coordination program helps you do just that. We strive to connect, support, and empower our members to reach their health goals. Care coordinators are trained professionals who know about many health conditions.
Get Help Finding Providers
A care coordinator can help you find a primary care provider (PCP) if you do not have one. Care coordinators can also help you find other health care providers to meet your care needs. This is helpful if you need to see several different doctors, or if you need a specialist that is hard to find.
Get Help When You Leave the Hospital
A care coordinator can help if you’ve just spent time in the hospital. They can help you set up your doctor visits after the hospital. They can also help to make sure you have the medical equipment and other things you need to recover.
Reach Your Health Goals
A care coordinator can help you learn to manage a chronic health condition, like diabetes, heart disease, or substance use recovery. Your care coordinator can also talk with you about your health goals and create a plan to meet them. They will support you as you make changes to take care of your health. They can also enroll you in digital health programs that give you regular support and reminders by text.

Your covered Health First Colorado (Colorado’s Medicaid Program) benefits and services are not changing. You can continue getting physical and behavioral health care. Your Health First Colorado eligibility has not changed.
RSV is a common, highly contagious infection. Most of the time, it will cause a mild, cold-like illness. Sometimes it will cause respiratory issues for your baby, including severe lung infections. Babies’ immune systems are still developing, so they may get infections more easily.
A medication called Beyfortus can help protect babies against serious lung infections from RSV and other types of infections. Beyfortus is an RSV vaccine that can boost your baby’s immune system to help them fight RSV. It is typically given as a shot in your baby’s thigh. Beyfortus is approved by the Food and Drug Administration (FDA). It is also recommended by the American Academy of Pediatrics (AAP).

We issue ID cards for members enrolled in the CHP+ State Managed Care Network and the CHP+ offered by Colorado Access programs. Health First Colorado issues cards for members enrolled in Health First Colorado (Colorado’s Medicaid program).
You may have to pay have a copay. This is what you pay when you get a covered health care service. The amount is based on the service.
If you have Health First Colorado, you may have a copay unless you are:
- Age 18 and younger.
- Age 18 to 25 and enrolled in former foster care.
- Pregnant
- Living in a nursing facility.
- Getting hospice care.
- An American Indian or Alaska Native.

You don’t need a referral. Some services, however, may require a prior authorization. It’s always a good idea to discuss your physical and behavioral health needs with your PCP.
Some CHP+ offered by Colorado Access members pay a co-pay for prescription medications. If you have a copay, the amount will be listed on your CHP+ card. If you have a copay, you will need to pay it before you can pick up the medication. If you are filling more than one prescription, you will need to pay the copay amount for each medication. If the retail price of the medication is less than your copay price, you will pay the retail price.
