| Dario Health | You will get a phone call inviting you to participate if you have diabetes and are eligible for the program. | You will get free devices to track your weight, blood pressure and glucose. You can also talk to your own health coach and get educational messages to improve diabetes control. | Opt in: Say YES to the Dario Health invitation phone call Opt out: Reply STOP to any text message |
| Text4Baby | You will get a phone call inviting you to participate if you are pregnant or recently had a baby and are eligible for the program. | You will get educational text messages on prenatal care, newborn health, and wellness tips directly to your phone. | Opt in: Say YES to the Text4Baby invitation phone call Opt out: Reply STOP to any text message |
