Purpose: Colorado Access is responsible for a variety of quality improvement initiatives as part of the Accountable Care Collaborative (ACC) program. These initiatives are outlined in the Colorado Access quality improvement plan that was submitted in July 2025. This report outlines the efforts to implement this plan over the previous fiscal year (FY) and/or calendar year (CY), the results of those efforts, and changes to Colorado Access’ approach.
