Health Programs

We can help make managing your health easier with things like care coordination to help you navigate your care, tools to help you manage your health day-to-day, and text messaging programs to keep you informed about your health.

If you have several health issues at the same time, or your care feels hard to manage, we can help.

Your doctor or care team may refer you, or we may see from your health records that you could use extra support.

If you are not eligible, our team will help connect you to a care coordination program that’s right for you.
You will work with a care coordinator who is here to support you. They will:
  • Learn about your needs.
  • Listen and ask questions to understand your health.
  • Help you make a care plan that fits you.
  • Work with your doctors and care team.
  • Connect you to helpful resources, like programs, services, or community support.

Care CoordinationAll members can get care coordination, even if you do not have a serious health condition. You may be offered care coordination if you need help understanding your care, getting appointments, or finding community resources. You will work with a care coordinator who is here to support you. They will:
  • Learn about your needs.
  • Listen and ask questions to understand your health.
  • Help you make a care plan that fits you.
  • Work with your doctors and care team.
  • Connect you to helpful resources, like programs, services, or community support.

Text4BabyYou will get a phone call inviting you to participate if you are pregnant or recently had a baby and are eligible for the program.You will get educational text messages on prenatal care, newborn health, and wellness tips directly to your phone.Opt in: Say YES to the Text4Baby invitation phone call. You can also sign up by following the instructions in the member newsletter.

Opt out: Reply STOP to any text message.
Text4KidsYou may be eligible if you are the parent or caregiver of a child ages 0–17 who is enrolled in Colorado Access. You will get a phone call inviting you to join the program.You will receive text messages with reminders and tips to support your child’s health, including checkups, immunizations, development, and school readiness.Opt in: Say YES to the Text4Kids invitation phone call. You can also sign up by following the instructions in the member newsletter.

Opt out: Reply STOP to any text message.
Text4HealthYou may be eligible if you are an adult (age 18 or older) enrolled in Colorado Access. You will get a phone call inviting you to participate.You will receive text messages with health tips and reminders, such as screening reminders and ideas for healthy living.Opt in: Say YES to the Text4Health invitation phone call. You can also sign up by following the instructions in the member newsletter.

Opt out: Reply STOP to any text message.
Text2QuitYou may be eligible if you are an adult (age 18 or older) who wants help quitting tobacco. You will get a phone call inviting you to join the program.You will receive encouraging text messages, tips, and reminders to help you quit smoking and manage cravings.Opt in: Say YES to the Text2Quit invitation phone call. You can also sign up by following the instructions in the member newsletter.

Opt out: Reply STOP to any text message.
Care4LifeYou may be eligible if you have diabetes and are enrolled in Colorado Access. You will get a phone call inviting you to participate.You will receive text messages with education, reminders, and goaltracking support to help manage your diabetes.Opt in: Say YES to the Care4Life invitation phone call. You can also sign up by following the instructions in the member newsletter.
