Provider Agreement Application

Thank you for your interest in becoming a contracted provider with Colorado Access. Colorado Access continuously monitors its provider network to ensure adequate access to health care services for our members, maintain appropriate network capacity so existing providers receive sufficient referrals, and prioritize meaningful support to our contracted providers.

Currently, contracting for the following specialties has been paused due to full network capacity:

• LCSW, LPC, LAC and LMFT providing adult outpatient services
• All Substance Use Disorder (SUD) providers serving adults, including residential and contracted SUD providers wanting to add programs
• Adult Peer Services

Exceptions may be considered for providers interested in participating in the Colorado System of Care (COSOC) who provide the following services: Enhanced Standardized Assessment (ESA); Enhanced Multisystemic Therapy (EMST); Enhanced Functional Family Therapy (RFFT); and Enhanced High-Fidelity Wraparound (EHFW). 

If you provide a subspecialty service, evidence-based practice, or services in a language other than English and wish to be considered for inclusion in the provider network, please complete an application. Each application will be carefully reviewed on an individual basis to ensure alignment with our network needs and priorities.
