Provider Agreement Application

Thank you for your interest in becoming a contracted provider with Colorado Access. We continuously monitors its provider network to ensure adequate access to health care services for our members, maintain appropriate network capacity so existing providers receive sufficient referrals, and prioritize meaningful support to our contracted providers.

Currently, contracting for the following specialties has been paused due to full network capacity:

• LCSW, LPC, LAC, and LMFT providers offering adult-focused outpatient services
• All substance use disorder (SUD) providers serving adults, including residential providers and contracted SUD providers requesting to add programs
• Adult-Focused Peer Services

If you provide a subspecialty service, evidence-based practice, or services in a language other than English and wish to be considered for inclusion in the provider network, please complete an application. Each application will be carefully reviewed on an individual basis to ensure alignment with our network needs and priorities.
